Guidelines for Completing Adult Award Nominations Requiring Board Approval

These guidelines are being provided as a tool to help you make sure that each of the necessary steps toward nominating a deserving person is followed and completed correctly.

The first step is to decide WHO you feel is a deserving candidate.

Then........

• Make sure that you have reviewed the requirements for the various awards.

• Determine the award that best fits the individual’s distinguished efforts.

• Check the Award Procedures Overview to review what you will need to submit for the nomination.

• Download and complete the Nomination For Adult Award Form.

• Check spelling and grammar. It is your responsibility to fill out this form completely.

• Choose individuals who will complete the Endorsement Form to support the nomination. Explain the importance of the clarity of the endorsement and give the endorsers the form to complete. It is very important that the guidelines are followed.

• It is suggested that you ask for more endorsements than you need. Then choose the endorsements that tell the nominee’s story most clearly.

• Give these individuals a deadline date by which they should have the completed endorsements back to you. Give yourself plenty of time to follow up on late arrivals.

• Call the endorsers to remind them of deadlines for their endorsements.

• Use the Award Criteria and these guidelines as a checklist to make sure that all of the pieces are completed.

• Submit all of the materials to the Council office by the deadline of FEBRUARY 1 by 5:00 pm to:
